Find the patient service center location that is most convenient for you. Be sure to check the hours for your chosen location.
If you have received paper lab orders from your healthcare provider, please bring them with you along with your insurance card and photo ID. *Note*: some providers submit electronic orders directly to our lab, so not all patients will need a paper copy.
If your testing requires special preparation, such as fasting, please be sure to follow all instructions as detailed by your healthcare provider. This will ensure your test results are accurate.
All locations are serving patients on a walk-in basis, and patients will be cared for efficiently and promptly.
